ロタウイルスワクチンで1型糖尿病が3割減

小児147万人超の医療保険データを解析

米·University of MichiganのMary A.M. Rogers氏らは、米国の小児147万人超の医療保険データを解析した結果、ロタウイルスワクチンを完全接種した小児は未接種の小児に比べ、その後の1型糖尿病発症率が33%低かったとSci Rep(2019; 9: 7727)に発表した。加えて、米国では未接種や不完全接種の小児が多い実態も明らかになった。

不完全接種では低下せず

米国では2006年に初めてロタウイルスワクチンが導入され、現在は2回接種の1価ワクチンと3回接種の5価ワクチンの2種類が用いられている。米疾病対策センター(CDC)は、生後15週以内に第1回目のワクチンを接種し、生後8カ月以内に全ての接種を終了(完全接種)するよう推奨している。

Rogers氏らは、全米規模の医療保険データベースClinformatics DataMartから、2001~17年に出生した小児147万4,535例のデータを抽出、解析した。解析対象の内訳は、ロタウイルスワクチンが導入された2006年以前(2001~05年)の出生児では未接種が54万6,972人、2006~17年に出生した児では完全接種が54万317人、不完全接種が14万646人、未接種が24万6,600人であった。

2006~17年に出生した完全接種児54万317人では中央値2.28年の追跡期間中に192例が、不完全接種児14万646人では中央値2.08年の追跡期間中に81例が、未接種児24万6,600人では中央値2.41年の追跡期間中に166例が1型糖尿病を発症した。1型糖尿病発症率は、10万人·年当たりそれぞれ12.2、20.5、20.6だった。

未接種児に比べ完全接種児では41%の1型糖尿病発症率の低下が認められた(発症率比0.59、95%CI 0.43~0.73)。一方、不完全接種児では、未接種児に比べて1型糖尿病発症率の低下は認められなかった(同0.99、0.75~1.30)。

Cox比例ハザード回帰分析の結果、2006~17年に出生したロタウイルスワクチン未接種の小児に比べ、完全接種した小児では1型糖尿病の発症リスクが33%低かった〔調整後ハザード比(HR)0.67、95%CI 0.54~0.83、P<0.001〕。

2006~17年に出生した完全接種児と2001~05年に出生した未接種児を追跡期間5年で比較したところ、1型糖尿病発症率は10万人·年当たり9.3と20.5で、完全接種児では未接種児に比べ1型糖尿病発症リスクが55%低下した(発症率比0.45、95%CI 0.37~0.56)。

特に5価ワクチンで低下

ワクチンの種類別に見ると、1価ワクチンを完全接種した小児では27%のリスク低下(HR 0.73、95%CI 0.50~1.07)が認められたのに対して、5価ワクチンを完全接種した小児では37%(同0.63、0.50~0.78)とリスクの低下度が大きかった。

未接種児に比べ完全接種児ではワクチン接種後60日以内の入院が31%低下した(発症率比0.69、95%CI 0.65~0.73)

また、ロタウイルスワクチンの接種率は地域による差が大きかった。

完全接種率が最も高かったのは中部諸州(イリノイ州、ミズーリ州、ネブラスカ州、カンザス州)の63.8%、最も低かったのはニューイングランド諸州の48.9%。未接種率が最も高かったのはニューイングランド諸州(34.8%)および太平洋沿岸諸州(32.8%)、最も低かったのは中部諸州(22.5%)であった。


ワクチン導入時期に一致した低下傾向

経時的な変化の解析では、2006年のロタウイルスワクチン導入と呼応するように、米国では2006~17年に0~4歳児における1型糖尿病発症率が年間3.4%低下していた。この結果は、オーストラリアでロタウイルスワクチン導入後に1型糖尿病発症リスクが14%低下したという最近の研究(JAMA Pediatr 2019; 173: 280-282)の結果と一致するという。

また、ロタウイルスは1型糖尿病患者で傷害される膵細胞と同様の細胞をロタウイルスが傷害するという実験結果とも一致している。

Rogers氏は「観察研究であるため因果関係を示すことはできない」と研究の限界を指摘した上で、「ロタウイルスワクチン接種により、小児の1型糖尿病発症リスクが低下することが示された。ロタウイルスワクチン接種は、1型糖尿病の予防に貢献しうる初の実用的な対策となる可能性がある」と結論。さらに、「毎年、完全接種児が10万人に達するごとに1型糖尿病の発症は8例減少する」と推算し、「ロタウイルスワクチンの普及に伴い、今後は1型糖尿病の新規発症が減少することが期待される。しかし、今回の研究結果に鑑みると、それは親が児にワクチンを接種させるかどうかにかかっている」と付言している。

Lower Incidence Rate of Type 1 Diabetes after Receipt of the Rotavirus Vaccine in the United States, 2001-2017.

Abstract

We evaluated whether rotavirus vaccination is associated with the incidence of type 1 diabetes among children. We designed a cohort study of 1,474,535 infants in the United States from 2001-2017, using data from a nationwide health insurer. There was a 33% reduction in the risk of type 1 diabetes with completion of the rotavirus vaccine series compared to the unvaccinated (95% CI: 17%, 46%). Completion of the pentavalent vaccine series was associated with 37% lower risk of type 1 diabetes (95% CI: 22%, 50%). Partial vaccination (incompletion of the series) was not associated with the incidence of type 1 diabetes. There was a 31% reduction in hospitalizations in the 60-day period after vaccination (95% CI: 27%, 35%) compared to unvaccinated children. Overall, there was a 3.4% decrease in incidence annually in children ages 0-4 in the United States from 2006-2017 which coincides with the vaccine introduction in 2006. We conclude that rotavirus vaccination is associated with a reduced incidence of type 1 diabetes. Rotavirus vaccination may be the first practical measure that could play a role in the prevention of this disease.

Rotavirus (RV) infection has been associated with the development of type 1 diabetes (T1D) in children.1 Rotavirus infection triggers pancreatic apoptosis in mice, and RV peptides display molecular mimicry with T-cell epitopes in pancreatic β-cell autoantigens.2 We hypothesized that if natural infection with RV was a causative factor in T1D, then RV vaccination would decrease the incidence of disease over time. Therefore, using publicly available data, we examined the incidence of T1D in Australian children before and after the oral RV vaccine was introduced to the Australian National Immunisation Program in 2007.
