距離1m以上で感染リスク8割以上減少、マスクでも減少

感染は主に飛沫核によって行われます。鼻腔から吸引されることで成立します。

一方、目からの感染はもっと大きな飛沫による感染で、直線的に結膜に飛び込んで成立します。

従ってゴーグルのような形態でなくても、メガネで十分です。


電車やタクシーの中では殆ど感染が起こっていません。マスク無しで、至近距離で、話す場面があることが最もリスクになります。

ウイルス伝播の予防に、人との物理的距離(physical distance、フィジカルディスタンス)1m以上の確保、フェイスマスクおよび保護眼鏡の着用が有効であることが、カナダ・マックマスター大学のDerek K. Chu氏らによるシステマティック・レビューとメタ解析の結果、示された。解析は、6大陸・16ヵ国で行われた172件の観察研究など患者総数2万5,697例を含むデータを包含して行われ、フィジカルディスタンスが1m以上の場合、感染リスクは約82%減少、フェイスマスクの着用は約85%減少することが示されたという。COVID-19を引き起こす新型コロナウイルス(SARS-CoV-2)は、人から人への感染が濃厚接触により拡大している。研究グループは医療機関および非医療機関(コミュニティなど)におけるウイルス伝播について、フィジカルディスタンス、フェイスマスク、保護眼鏡の影響を明らかにするため本検討を行った。Lancet誌オンライン版2020年6月1日号掲載の報告。


フィジカルディスタンスなどの影響をシステマティック・レビューとメタ解析で検証

 システマティック・レビューとメタ解析は、フィジカルディスタンス、フェイスマスク、保護眼鏡の影響を明らかにするために、PubMedやEmbaseなど世界保健機関(WHO)が特定する21の標準的データベースと、COVID-19の特異的データベースを基に、重症急性呼吸器症候群を引き起こすSARS-CoV-2とβコロナウイルス、および中東呼吸器症候群(MERS)に関するデータを集めて行われた。


 2020年5月3日時点でシステマティック・レビューを実施。言語は問わなかったが、比較試験であることを適格条件とし、また、試験の容認度や柔軟性、資源活用性、公平性の文脈的要因を確認した。そのうえで試験結果をスクリーニングしデータを抽出、バイアス・リスクについては2度評価した。


 頻度論的統計、ベイズ・メタ解析、ランダム効果メタ回帰解析を行った。エビデンスの確実性については、Cochrane法とGRADEアプローチで評価した。


フィジカルディスタンス1m以上で0.18倍、マスクで0.15倍、保護眼鏡で0.22倍に

 検索により、6大陸にわたる16ヵ国からの観察研究172件を特定した。無作為化試験はなかった。


 医療現場・非医療現場で行われた比較試験であるとの適格条件を満たした44件(患者総数2万5,697例)を対象にメタ解析を行った。


 感染リスクは、フィジカルディスタンスが1m未満の場合に比べ1m以上の場合は、大幅に低下した(被験者総数1万736例、統合補正後オッズ比[OR]:0.18[95%信頼区間[CI]:0.09~0.38]、群間リスク差[RD]:-10.2%[95%CI:-11.5~-7.5]、中等度の確実性)。物理的間隔が広がるほど、ウイルスの感染防御は増大した(1m増の相対リスク[RR]変化:2.02、交互作用のp=0.041、中等度の確実性)。


 フェイスマスクの着用も、感染リスクの大幅な減少につながった(被験者総数2,647例、統合補正後OR:0.15[95%CI:0.07~0.34]、RD:-14.3%[95%CI:-15.9~-10.7]、低度の確実性)。とくに、N95マスクや類似の防毒マスクは、使い捨てサージカルマスクや類似のマスク(再利用できる12~16層の綿マスクなど)と比べ、感染予防効果との関連性が強かった(交互作用p=0.090、事後確率95%超、低度の確実性)。


 保護眼鏡の使用も感染リスクの低減につながった(被験者総数3,713例、統合補正後OR:0.22[95%CI:0.12~0.39]、RD:-10.6%[95%CI:-12.5~-7.7]、低度の確実性)。


 未補正試験やサブグループ解析、感度解析でも、同様の結果が得られた。


 著者は、「システマティック・レビューとメタ解析の結果は、1m以上のフィジカルディスタンスを支持するもので、政府が取り組むべきモデルおよび接触者追跡の定量的推定値が明らかになった。解析結果に基づき、公共の場および医療機関でのフェイスマスク、防毒マスク、保護眼鏡の適切な使用を広報しなければならない」と述べるとともに、「今回の介入のエビデンスをより明らかにするために、頑健な無作為化試験を行う必要があるが、解析結果は現時点で得られる最善のエビデンスであり、中間的なガイダンスとして利用可能と思われる」とまとめている。

Abstract

BACKGROUND : Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) causes COVID-19 and is spread person-to-person through close contact. We aimed to investigate the effects of physical distance, face masks, and eye protection on virus transmission in health-care and non-health-care (eg, community) settings.


METHODS :

We did a systematic review and meta-analysis to investigate the optimum distance for avoiding person-to-person virus transmission and to assess the use of face masks and eye protection to prevent transmission of viruses. We obtained data for SARS-CoV-2 and the betacoronaviruses that cause severe acute respiratory syndrome, and Middle East respiratory syndrome from 21 standard WHO-specific and COVID-19-specific sources. We searched these data sources from database inception to May 3, 2020, with no restriction by language, for comparative studies and for contextual factors of acceptability, feasibility, resource use, and equity. We screened records, extracted data, and assessed risk of bias in duplicate. We did frequentist and Bayesian meta-analyses and random-effects meta-regressions. We rated the certainty of evidence according to Cochrane methods and the GRADE approach. This study is registered with PROSPERO, CRD42020177047.


FINDINGS :

Our search identified 172 observational studies across 16 countries and six continents, with no randomised controlled trials and 44 relevant comparative studies in health-care and non-health-care settings (n=25 697 patients). Transmission of viruses was lower with physical distancing of 1 m or more, compared with a distance of less than 1 m (n=10 736, pooled adjusted odds ratio [aOR] 0·18, 95% CI 0·09 to 0·38; risk difference [RD] -10·2%, 95% CI -11·5 to -7·5; moderate certainty); protection was increased as distance was lengthened (change in relative risk [RR] 2·02 per m; pinteraction=0·041; moderate certainty). Face mask use could result in a large reduction in risk of infection (n=2647; aOR 0·15, 95% CI 0·07 to 0·34, RD -14·3%, -15·9 to -10·7; low certainty), with stronger associations with N95 or similar respirators compared with disposable surgical masks or similar (eg, reusable 12-16-layer cotton masks; pinteraction=0·090; posterior probability >95%, low certainty). Eye protection also was associated with less infection (n=3713; aOR 0·22, 95% CI 0·12 to 0·39, RD -10·6%, 95% CI -12·5 to -7·7; low certainty). Unadjusted studies and subgroup and sensitivity analyses showed similar findings.


INTERPRETATION :

The findings of this systematic review and meta-analysis support physical distancing of 1 m or more and provide quantitative estimates for models and contact tracing to inform policy. Optimum use of face masks, respirators, and eye protection in public and health-care settings should be informed by these findings and contextual factors. Robust randomised trials are needed to better inform the evidence for these interventions, but this systematic appraisal of currently best available evidence might inform interim guidance.


身体的距離を取ることはとても有効で、特に換気の良い空間では有効です。

一方である程度の距離を取っても、換気が悪く声を出す人の空間では効果は限定的になってしまいます。

普通、欧米では Physical distance(身体的距離)と言います。Social distance (社会的距離) ではありません。社会的距離は人を孤立させ、精神を蝕みます。私はSocial distance という言葉が嫌いです。マスコミの英語力が低いために誤った言葉が定着したことが残念です。

身体的距離のCoVID-19に対する予防効果は限定的

世界各国で行ったSocial distancingの効果が、SARS-COV2のようなステルス性の高いウイルスの場合、どの程度有効なのかが分からないまま、スペイン風邪やSARS-COV1やMERSでの経験から行われた。

(どれも過去にはある程度、効果を発揮した手法だ。)


CoVID-19の流行初期の移動制限は有効だが、ある程度広がると、公共交通機関の封鎖の追加的効果は得られない。


クラスターが殆どできない学校の封鎖は(インフルエンザとは違い)殆ど無意味だろう。


潤沢なPCRリソースに基づいたクラスター追跡が完全にできている国以外では、最早抑え込むことは不可能になっている。

この経済的ダメージが大きい手法によって、たった13%しか減らせないのなら、手洗いと屋内のUniversal masking だけで十分ではないかと私は思う。


殆ど患者がいない地域や施設を中心に隔離して、貧弱なPCRのリソースをリスクが高い集団に適応するしか、方法がないように思うのに、未だに健康な若いスポーツ選手に行っているのは、理解し難い。

新型コロナウイルス感染症(COVID-19)への介入としての身体的距離(フィジカルディスタンス)の確保は、COVID-19の発生を13%抑制し、移動制限(封鎖)は実施時期が早いほうが抑制効果は大きいことが、英国・オックスフォード大学のNazrul Islam氏らが世界149ヵ国を対象に実施した自然実験で明らかとなった。研究の成果はBMJ誌2020年7月15日号に掲載された。COVID-19に有効な治療レジメンやワクチンのエビデンスがない中で、感染を最小化するための最も実践的な介入として、身体的距離の確保が推奨されてきた。この推奨の目標には、公衆衛生や保健サービスにおけるCOVID-19による負担の軽減や、この疾患の予防と管理のための時間の確保も含まれている。これらの介入の有効性に関するこれまでのエビデンスは、主にモデル化研究に基づいており、実地の患者集団レベルの有効性のデータは世界的に少ないという。


分割時系列解析による自然実験の結果をメタ解析で統合

 研究グループは、身体的距離の確保とCOVID-19の世界的な発生の関連を評価する目的で、国別に分割時系列解析による自然実験を行い、得られた結果をメタ解析で統合した(筆頭著者は、オックスフォード大学ナフィールド公衆衛生学科[NDPH]から給与の支援を受けた)。


 解析には、欧州疾病予防管理センターから毎日報告されるCOVID-19患者のデータと、オックスフォードCOVID-19政策反応追跡(Oxford COVID-19 Government Response Tracker)からの身体的距離の確保に関するデータを用いた。対象は、2020年1月1日~5月30日の期間に、5つの身体的距離の確保(学校閉鎖、職場閉鎖、公共交通機関閉鎖、大勢の集まりや公的行事の規制、移動制限[封鎖])のうち1つ以上を、少なくとも7日間実施し、30例以上のCOVID-19患者が発生した国であった。


 主要アウトカムは、身体的距離の確保による介入の導入前後のCOVID-19発生率比(IRR)とし、5月30日または介入後30日目のいずれか先に到達した日のデータを用いて推定した。IRRは、変量効果によるメタ解析を用いてすべての国のデータを統合した。


公共交通機関閉鎖を除く4つと、5つ全部で抑制効果に差はない

149ヵ国が1つ以上の身体的距離確保による介入を行っていた。学校閉鎖のみのベラルーシと、学校閉鎖と大勢の集まりの規制のみのタンザニアを除く国が、3つ以上の介入を実施していた。5つ全部を実施したのが118ヵ国、4つが25ヵ国(日本は公共交通機関閉鎖以外の4つを実施)、3つが4ヵ国で行われていた。


 身体的距離確保の実施により、COVID-19の発生が全体で平均13%低下した(IRR:0.87、95%信頼区間[CI]:0.85~0.89、p<0.001、149ヵ国)。日本は、4つの介入の実施によりCOVID-19の発生が6%抑制された(0.94、0.90~0.99)。


 介入前と比較したCOVID-19発生の低下率は、5つの身体的距離確保を実施した国(統合IRR:0.87、95%CI:0.85~0.90、118ヵ国)と、4つを実施した国(0.85、0.82~0.89、25ヵ国)で類似していた。3つを実施した国(0.88、0.77~1.00)では、COVID-19発生率の変化が小さかったが、適用されたのは4ヵ国のみだった。


 学校閉鎖、職場閉鎖、大勢の集まりの規制、移動制限が実施された場合のCOVID-19発生の低下(統合IRR:0.87、95%CI:0.84~0.91、32ヵ国)は、これに公共交通機関閉鎖を加えて5つすべての介入を行った場合(0.85、0.82~0.88、72ヵ国)とほぼ同等で、付加的な抑制効果は得られなかった。また、学校閉鎖、職場閉鎖、大勢の集まりの規制が実施された場合のCOVID-19発生の抑制効果もほぼ同様であった(0.85、0.81~0.89、11ヵ国)。


 介入の順序に関しては、移動制限を早期に実施した場合(統合IRR:0.86、95%CI:0.84~0.89、105ヵ国)のほうが、他の身体的距離確保による介入後に遅れて移動制限を行った場合(0.90、0.87~0.94、41ヵ国)に比べ、COVID-19発生の抑制効果が大きかった。




Physical distancing interventions and incidence of coronavirus disease 2019: natural experiment in 149 countries.

Abstract

OBJECTIVE :

To evaluate the association between physical distancing interventions and incidence of coronavirus disease 2019 (covid-19) globally.


DESIGN :

Natural experiment using interrupted time series analysis, with results synthesised using meta-analysis.


SETTING :

149 countries or regions, with data on daily reported cases of covid-19 from the European Centre for Disease Prevention and Control and data on the physical distancing policies from the Oxford covid-19 Government Response Tracker.


PARTICIPANTS :

Individual countries or regions that implemented one of the five physical distancing interventions (closures of schools, workplaces, and public transport, restrictions on mass gatherings and public events, and restrictions on movement (lockdowns)) between 1 January and 30 May 2020.


MAIN OUTCOME MEASURE :

Incidence rate ratios (IRRs) of covid-19 before and after implementation of physical distancing interventions, estimated using data to 30 May 2020 or 30 days post-intervention, whichever occurred first. IRRs were synthesised across countries using random effects meta-analysis.


RESULTS :

On average, implementation of any physical distancing intervention was associated with an overall reduction in covid-19 incidence of 13% (IRR 0.87, 95% confidence interval 0.85 to 0.89; n=149 countries). Closure of public transport was not associated with any additional reduction in covid-19 incidence when the other four physical distancing interventions were in place (pooled IRR with and without public transport closure was 0.85, 0.82 to 0.88; n=72, and 0.87, 0.84 to 0.91; n=32, respectively). Data from 11 countries also suggested similar overall effectiveness (pooled IRR 0.85, 0.81 to 0.89) when school closures, workplace closures, and restrictions on mass gatherings were in place. In terms of sequence of interventions, earlier implementation of lockdown was associated with a larger reduction in covid-19 incidence (pooled IRR 0.86, 0.84 to 0.89; n=105) compared with a delayed implementation of lockdown after other physical distancing interventions were in place (pooled IRR 0.90, 0.87 to 0.94; n=41).


CONCLUSIONS :

Physical distancing interventions were associated with reductions in the incidence of covid-19 globally. No evidence was found of an additional effect of public transport closure when the other four physical distancing measures were in place. Earlier implementation of lockdown was associated with a larger reduction in the incidence of covid-19. These findings might support policy decisions as countries prepare to impose or lift physical distancing measures in current or future epidemic waves.
